Report details flaws in Army's handling of PTSD

SEATTLE (AP) ? An Army report released Friday finds the service still has trouble diagnosing and treating soldiers for post-traumatic stress disorder, despite more than doubling its number of military and civilian behavioral health workers over the past five years.
Confusing paperwork, inconsistent training and guidelines, and incompatible data systems have hindered the service as it tries to deal with behavioral health issues, the report said. It's a crucial issue: After a decade of war, soldier suicides outpace combat deaths.
Last May, the Army commissioned a task force to conduct a sweeping review of how it evaluates soldiers for mental health problems at all its facilities. The review came under pressure from Democratic Sen. Patty Murray, of Washington, who was upset to learn that hundreds of soldiers at Madigan Army Medical Center south of Seattle had had their PTSD diagnoses reversed by a forensic psychiatry team, resulting in a potential cut to their benefits and questions about whether the changes were made to save money.
About 150 of those soldiers eventually had their diagnoses restored.
"I am pleased that the Army completed this review and has vowed to make fixes over the next year, though I am disappointed it has taken more than a decade of war to get to this point," Murray said in a statement. "Many of the 24 findings and 47 recommendations in this report are not new. Creating a universal electronic health record, providing better rural health access, and standardizing the way diagnoses are made, for instance, have been lingering problems for far too long. Our service members and their families deserve better."
The report noted that the Army had made strides in some areas, including cutting how long it takes soldiers to obtain a disability evaluation and publishing a guide to the process.
On a conference call with reporters, Army brass emphasized that many of the report's recommendations are already being put into effect. For example, over the past year the Army has been assigning behavioral health workers to brigade combat teams so soldiers will feel more familiar with them and more comfortable about getting help, said Lt. Gen. Patricia D. Horoho, who heads the Army's Medical Command.
Horoho also stressed that there was no evidence that malice motivated the altered diagnoses at Madigan; rather, the changes amounted to difference of opinion, she said.
The task force interviewed 750 people stationed around the globe, conducted listening sessions with 6,400 others and reviewed more than 140,000 records. The Medical Command reviewed diagnoses for all soldiers evaluated for behavioral health problems from October 2001 until last April.
Since September 2001, the report found, 4.1 percent of all soldiers deployed wound up in the disability system with a behavioral health diagnosis such as PTSD or traumatic brain injury.
Nationwide, the report said, 6,400 soldiers had behavioral health diagnoses "adjusted" by medical evaluation boards, with approximately equal numbers having PTSD added as a diagnosis and removed as a diagnosis.
Two locations where medical evaluation boards are held had slightly higher rates of diagnosis changes than the Army-wide average ? Fort Polk in Louisiana and Fort Irwin in California, Horoho said. Cases from those locations are being reviewed to ensure no soldiers were improperly affected, but part of the reason for the higher rates may be because those bases rely heavily on civilian health workers, she said.
Last year the Army ? and the military as a whole ? suffered the highest number of suicides ever recorded, prompting then-Defense Secretary Leon Panetta to declare it an epidemic. The Army had 183 suicides among active-duty soldiers, up from 167 in 2011, and the military as a whole had 350 suicides, up from 301 the year before.
Among the problems the report documented was that Army bases don't have a person on site dedicated to overseeing behavioral health issues, despite the many problems they can cause: suicide, alcohol abuse, drug abuse, and child and spouse abuse. Each installation needs someone with a view of all those programs to make recommendations to the commander, the report said.
Army Secretary John M. McHugh said in a statement that the Army will work to place behavioral health experts "at the command and installation levels to provide better consultation, guidance, coordination and recommendations to improve behavioral health care for our soldiers."
The task force found that of the soldiers surveyed, 37 percent had never received any information about the Army's disability evaluation system or had to seek the information out on their own. It also said it was confusing and inefficient for troops to navigate the vastly different disability systems maintained by the Army and the Veterans Administration.
The Army and VA plan to have a joint disability system, by which health care providers in either organization will have access to records, by 2017.
"Some changes can be made immediately," McHugh said. "Others will require more time and coordination. Importantly, this report reviewed our systems holistically ? recommending not only short-term solutions, but longer term, systemic changes that will make care and treatment of our soldiers and family members more effective."

Players' association gives OK to NHL realignment
NEW YORK (AP) ? The NHL moved a step closer to realignment Thursday as the players' association approved a proposed plan that is set to go into effect next season.
Now that the league has gotten the go-ahead from union chief Donald Fehr, the NHL will only need to get approval from team owners to put it in place.
"The NHL Players' Association confirmed to us today that it has consented to a revised plan for realignment, effective for the 2013-14 season," NHL Deputy Commissioner Bill Daly said in a statement. "Our next step will be to bring the proposed plan for realignment to the NHL board of governors for its consideration."
Fehr signed off on the plan after discussions with the union's executive board and said the realignment issue will be "re-evaluated following the 2014-15 season."
No official details of the changes have been released by the NHL, but it is widely believed that teams such as Detroit and Columbus will be put into more travel-friendly divisions in the Eastern time zone, and the Winnipeg Jets ? formerly the Atlanta Thrashers ? will leave the Southeast Division for a spot in the Western Conference.
Up until now, there has been an even split of teams with 15 in each conference. Under the new plan, the East will have 16 clubs in it ? making playoff qualification a bit tougher.
The two divisions out West will contain seven teams each. The East divisions will both contain eight.
Each conference will still have eight teams in the playoffs but the setup will be slightly different.
Instead of the current system, under which the top eight teams in each conference make the playoffs, the new plan will award spots to the top three teams in each division along with the next two teams with the best records as wild cards.
The new Atlantic Division in the East will be comprised of Carolina, Columbus, New Jersey, New York's Rangers and Islanders, Philadelphia, Pittsburgh and Washington. The Central Division will contain Boston, Buffalo, Detroit, Florida, Montreal, Ottawa, Tampa Bay and Toronto.
Out West, the new Midwest Division will feature Chicago, Colorado, Dallas, Minnesota, Nashville, St. Louis and Winnipeg. The Pacific Division will be made up of Anaheim, Calgary, Edmonton, Los Angeles, Phoenix, San Jose and Vancouver.

A couple of weeks ago, Jeff and I spent a morning running around after our children during a carnival hosted at their school. As hour number three approached, my capacity for other people?s children rapidly reaching code red zone, I pulled out my phone and hopped on Twitter for a few minutes. Of course, those few minutes happened to coincide with my bumping into one of my children?s teachers.
?
After she greeted me, she gently pulled the phone out of my hand and whispered ?tsk, tsk? as she shook her head. My face turned red as she gestured to the surrounding children whom I clearly should have been watching instead. My phone shamefully remained in my coat pocket for the rest of the afternoon, while I found other ways to occupy the time, none of which included actually playing with the kids who blissfully ignored my presence while they ran around with their friends.

I forgot about the whole interaction (or, rather blocked it out) until I came across a post yesterday,?Dear Mom on The iPhone.?It begins?

It?goes on and on and on about the precious moments that a mother is missing while immersed in technology. ?Now you are pushing your baby in the swing. She loves it! Cooing and smiling with every push. You don?t see her though, do you? Your head is bent, your eyes on your phone as you absently push her swing.?
?
Bad, bad mother, the comments preach. How selfish! How self-absorbed! How dare a mother?absentmindedly push a swing when she could be relishing every single back and forth motion. What is our society coming to?
?
Well, I am that mother at the park on her iPhone, thank you very much. I?m the one who gets scowled at and pointed to and written about. Sometimes it?s the park, others it?s an indoor playzone or maybe it?s a birthday party. If I?m out with my kids, and they are entertained, it?s not uncommon that my iPhone is entertaining me. But that fact doesn?t?make me a bad mom. In fact, I?d argue that it helps make me a better one.
?
Checking in on Twitter or?Facebook?allows me to collect myself and maintain a sense of humor about things that might otherwise set me off. It?s kind of the social media immersed mother?s version of a long drag on a cigarette. It helps ground me and gain perspective.?The permanent marker covered Evan a few years ago would have been far more upsetting than amusing were it not for the ensuing hilarity in?Facebook?comments. Having my friends and community a simple click away is a much needed break at the very least, and a near lifesaver at the most.
?
I work from home,?and part of working from home (FYI,?Marissa Mayer) means taking that work with me, wherever my day may go. I am fortunate to have the freedom and flexibility to bring my kids into school every day and pick them up at three and spend the afternoon and evening hours with them. And I treasure that ability. If it means having to respond to e-mails or follow up on things while the rest of the working world is still behind a desk, I don?t see that as a problem. Would it be better to get a traditional desk job and have a nanny caring for them all day and night?
?
Besides, being on my phone in public, at places like sports practice where the kids are surrounded by friends and the park where they can run around and play, makes it more likely that I?ll be off of the phone when I?m home alone with them.
?
And maybe, I?m on my phone at the park because I don?t really feel like engaging with that preachy looking mother who, if not judging me for my phone use, would most certainly find something else about me to be appalled by.
?
I would never, ever claim to be a perfect mother. I have moments of stellar mothering and moments of complete crappiness, and they are usually separated by mere seconds.?At the end of the day, I strive for two things: 1. To make my children feel loved, and 2. To have the proud parenting moments outweigh the regrettable ones. Adding ?always place undivided attention on my children 24/7? to the list really wouldn?t benefit my children and it sure as hell wouldn?t benefit me.
?
One small snippet that someone happens to witness at a park or a?restaurant or in a parking lot hardly paints a thorough picture of?any?family, so judging based on what you happen to catch in a single moment is laughable. Almost as laughable as the notion of taking my kids to a bounce zone and having my eyes glued to them jumping up and down for four hours straight.

Astronomers find 'lost' supernova
Mar. 7, 2013 ? The star Eta Carinae is ready to blow. 170 years ago, this 100-solar-mass object belched out several suns' worth of gas in an eruption that made it the second-brightest star after Sirius. That was just a precursor to the main event, since it will eventually go supernova.
Supernova explosions of massive stars are common in spiral galaxies like the Milky Way, where new stars are forming all the time. They are almost never seen in elliptical galaxies where star formation has nearly ceased. As a result, astronomers were surprised to find a young-looking supernova in an old galaxy. Supernova PS1-12sk, discovered with the Pan-STARRS telescope on Haleakala, is rare in more ways than one.
"This supernova is one-of-a-kind," said Nathan Sanders of the Harvard-Smithsonian Center for Astrophysics (CfA), lead author of the discovery paper. "And it's definitely in the wrong neighborhood."
Based on the presence of helium and other features, PS1-12sk is classified as a very rare Type Ibn supernova -- only the sixth such example found out of thousands of supernovae. Although the origin of this supernova type is unclear, the most likely cause seems to be the explosion of a massive star that previously ejected massive amounts of helium gas, much like Eta Carinae's Homunculus Nebula.
That origin was supported by the fact that the five previous Type Ibn supernovae were all found in galaxies like the Milky Way that are actively forming stars. (Since massive stars don't live long, they don't stray far from where they are born before exploding.)
PS1-12sk is different. It was found on the outskirts of a bright elliptical galaxy located about 780 million light-years from Earth. The site of the explosion shows no signs of recent star formation, and a supernova from a massive star has never before been seen in a galaxy of this type.
"It could be that we simply got very lucky with this discovery. But luck favors the prepared," said second author Alicia Soderberg of the CfA.
The finding suggests that the host galaxy might be hiding a star factory, allowing it to form massive stars where none were expected. Alternatively, PS1-12sk might have an entirely different origin such as a collision of two white dwarfs, one of which was helium-rich.
"Is this a runaway star from another star formation site? Is it a very local bit of star formation? Is it a different way for such a supernova to occur? None of these seems very likely so we have a real puzzle," said co-author John Tonry (University of Hawaii Institute for Astronomy).
The research has been submitted for publication in The Astrophysical Journal.
